Lived experience
We are committed to embedding consumers’ voices and experiences in service design and delivery.
Insights gathered through our annual Health Needs Assessments highlight emerging health concerns and areas for service improvement. Listening to people’s experiences creates an even more holistic picture of our community’s diverse needs. This year, our consumer focus included First Nations people, older people, people with lived experience of mental illness and culturally and linguistically diverse people. This led to the design of true person-centred programs, and culturally appropriate evaluation models.

Collaboration
We partner with health professionals, consumers, service providers and government agencies to enable better health outcomes for individuals, families and communities.
Purposefully and continuously engaging with stakeholders provides opportunities for collective problem-solving and improved integration across the sector. Many initiatives throughout the year saw several organisations and professionals collaborate during the design and implementation phases. Significant reforms, such as the four new Priority Primary Care Centres in south east Melbourne, were highly successful due to sustained effort and rapport established between SEMPHN, ambulance, emergency departments and general practices. Collaboration is a core value at SEMPHN and embraced by our staff.

Innovation
We are playing a purposeful, influential and innovative role in local and regional health service reform and integration.
Being able to respond to the complex and rapidly changing health landscape is integral to the success of primary health care providers. We regularly meet with providers to discuss challenges within their services and community and work with them to develop innovative and sustainable solutions. Over the past year, we have commissioned many innovative initiatives and services across mental health, chronic disease, alcohol and other drugs, aged care, cancer prevention and workforce development.
